Midwest Sound entertainers have extensive experience playing music for Bar and Bat Mitzvahs, Quinceañeras, School Dances, Corporate Gatherings and many other events. Once you complete and submit the planner an on-screen confirmation will let you know your planner was received. To access and complete the form please click here or the button below.

GENERAL INFORMATION

Provide us with information regarding your upcoming event (date, venue, times, etc.). 
This information will assist us in assigning the perfect, experienced entertainer that best fits
your event needs.

Here at Midwest Sound, ensuring your event is memorable and entertaining is our #1 priority. To that end, let us know a bit more as to what type of entertainer you would like. From their participation level to any special instructions/needs.
 



MUSIC AND EVENT HIGHLIGHTS

The final sections within the planner requests a bit of information regarding the type of
music you would like played at the event as well as outline plans for speeches, special
dances, events, etc.

Click Save after completing the form. 
Click Submit when you are ready to send.
